I like your idea that she looked like his mom. The AI did seem really upset that John's imagination kicked up SEVERELY dark scenarios and that would be a way to pay him back a little. If I could write, I would write dream tag of the AI as his Mom pointing out that she couldn't have raised such a kind, loving son if she had been the type to be ashamed of her gay son.
Some people seem to find John's "self-torture" unreasonable, but for a sensitive and compassionate commander, the scenario is not unreasonable.
He would have seen aftermaths of, and heard of, much worse in Afghanistan. Even Kolya was never as bad as some of the Soviet-inspired atrocities the Taliban and other Afghani groups get up to.
